SHACC is Open All Summer! (906)632-5690 It’s Time to Schedule a Physical Examination! All students of Sault Area High School and Career Center, Sault Middle School, and Malcolm School are eligible for a comprehensive physical examination every year at the Sault Health Adolescent Care Center. The summer is a great time to get a physical especially for students who participate in sports. A current-year physical for the sport card is one given on or after April 15 of the previous school year. Most student athletes get their physicals at the SHACC in the late spring or early summer to be ready for a full school-year of sports. Schedule an appointment today! The SHACC comprehensive physical examination includes height/weight/body mass index, health appraisal, immunization review, vision and hearing screening, hemoglobin if indicated, risk assessment, and the completion of a sport card. Insurance including Medicaid is accepted. The maximum fee for a comprehensive physical not covered by insurance is $10. Students without insurance will be offered Medicaid information. No student is denied services due to inability to pay. General medical and counseling services are available all summer. Call (906)632-5690 for an appointment. Visit our website www.chippewahd.com/shacc If you need a work permit, please keep the following in mind: ✔️ The student must be present to obtain the work permit. ✔️ Students who do not attend Sault Area Public Schools must bring identification (birth certificate or photo ID). ✔️ The employer must complete their portion of the work permit before it can be signed and issued. Back to School Packet & Education Benefit Form We understand this is a busy time, but we are asking you to take time to complete the Back-to-School Annual Verification Packet, and the Education Benefit Form (formerly known as the Free & Reduced Lunch form). Please complete these forms before September 30th. This verification is for all students. Back to school packet completion allows parents/guardians to review and update their household and student information and provides the school with state required information. By completing the Education Benefit Form, you are providing our schools with the opportunity to receive access to funding for educational programs that our students benefit from. You may also find a link to the Education Benefit Form below. You can complete these forms electronically in your MIStar Parent Portal account via the mobile app or website (Parent Portal (oakland.k12.mi.us)). Back -To-School Packet: Computer Back-To-School Packet: App Education Benefit Form Friendly reminder to parents & guardians of Kindergarteners, incoming 7th grade students, and all new incoming students to Sault Area Public Schools. Please have your child's immunizations caught up for the beginning of the new 2026/2027 school year. This can be done at the Chippewa County Health Department, 906-635-1566, or the SHACC, 906-632-5690, or your health provider. Immunizations are to be up to date by the time school starts.
