Day of Giving Back/Premier Health Vaccination Clinic

Happening During 937 Day of Giving Back! Give your child a healthy start to the school year with required school vaccines at NO COST for families who qualify for the Vaccine for Children (VFC) Program. This clinic serves VFC-eligible K-12 students ONLY. Privately insured children cannot be vaccinated at this event. Parent/Guardian Information Plan to remain onsite for at least 15 mins after receiving vaccine(s). Bring child’s immunization record (if applicable), Medicaid card (if applicable), parent/guardian photo ID, proof of eligibility if needed. Vaccines Available DTaP/Tdap (Diphtheria, Tetanus, & Pertussis) Polio (IPV) MMR (Measles, Mumps, & Rubella) Varicella (Chickenpox) Hepatitis B Meningococcal (Grades 7-12, as needed) Who Can Get Vaccinated?
